This form is used to change your address and/or phone number.
This form is used to change your name or social security number. Please keep in mind that you will be asked to provide proof of the changes.
This form is used to verify your status at DeVry. It can be used for health insurance or for other services that need you to provide proof that you are or were a student at DeVry.
This form is used to get permission to transfer from one program to another. Please keep in mind that you must meet the requirements for the program you want to transfer to and be approved by the program deans.
DeVry students who leave their program for one or two semesters or who transfer to another program or DeVry location must sign this addendum prior to resuming or transferring. Students who leave the program for 3 or more semesters will be asked to execute a new agreement prior to resuming. Students who have signed an enrollment agreement prior to a tuition increase sign this addendum at registration to acknowledge receipt of notification of the increase.
This form is completed by DeVry students who wish to request that all paperwork required to transfer from DeVry Arlington to another DeVry location be forwarded, including but not limited to: copies of their Application for Admission, Enrollment Agreement, transcripts and all Academic and Financial Aid records.
This form is used to request an official or unofficial transcript. Please keep in mind that there is a $5.00 fee for each transcript. You may mail in this form with a $5.00 check or money order for each transcript requested, or you can process it on campus. Faxed requests are not accepted.
As of July 1, 2003, transcripts are $5.00 each.
If you cannot find the form you are looking for, you must obtain the form in person from the Registrar's Office in Academics.
